RE: CEASE AND DESIST — UNLAWFUL ACTS OF SEXUAL HARASSMENT AND GENDER-BASED HARASSMENT
Dear ________:
This letter serves as FORMAL NOTICE AND DEMAND that you immediately CEASE AND DESIST from committing acts that are unwelcome, sexual, and/or gender-based in nature directed against me, the undersigned, ________, of legal age and presently residing at ________.
§ 1. Capacity of the Parties. I am a bona fide student/trainee at ________ (the “Institution”). You hold the position of ________ within the Institution, a position which carries authority, influence, ascendancy, and/or moral influence over me as a student/trainee.
§ 2. The Unlawful Conduct. By reason of your conduct, the environment at ________ has become intimidating, hostile, and offensive to me, to the point that my access to education, training, and the use of the premises has been materially impaired. The specific acts complained of are the following:

§ 3. Resulting Harm. The foregoing acts have directly caused me the following injury and prejudice:

The said acts are evidenced by the documents and other materials annexed hereto and made integral parts of this letter.
§ 5. Demand. ACCORDINGLY, I hereby DEMAND that you, within ________ from your receipt hereof:
(a) immediately and permanently CEASE AND DESIST from committing any and all of the unwelcome, sexual, and/or gender-based acts described herein, and from any act of similar or related nature against me;
(b) refrain from any direct or indirect contact, communication, approach, surveillance, or conduct directed toward me, my family, or my associates, whether in person, in writing, electronically, or through any other means; and
(c) refrain from any act of retaliation, intimidation, or reprisal of whatever nature against me by reason of this letter or any complaint I may file.
§ 7. Reservation of Rights. Nothing in this letter shall be construed as a waiver of, and I expressly reserve, all my rights, claims, causes of action, and remedies under the law and equity, all of which are hereby reserved without prejudice.
