RE: NOTICE OF CANCELLATION OF SUBSCRIPTION AND REVOCATION OF AUTHORITY FOR AUTOMATIC/RECURRING BILLING
§1. Introduction
I, ________, the holder of the subscription account bearing reference/account number ________ maintained with your company ("the Company"), write to formally notify you of the cancellation of my subscription and the immediate revocation of any and all standing authority, mandate or consent previously granted to the Company to debit, charge or withdraw sums from my account on a recurring or automatic basis.
§2. Particulars of the Subscription
2.1 I subscribed to ________ ("the Service"), in respect of which the Company has been billing me the sum of ₦________ on a ________ basis.
2.2 The said charges have been applied to my ________ bearing the following identifying details: ________, held with ________.
§3. Cancellation and Revocation of Mandate
3.1 With effect from the date of this letter, I hereby cancel my subscription to the Service in accordance with the applicable terms and conditions of the subscription agreement between the Company and me.
3.2 I further withdraw and revoke, with immediate effect, the recurring payment mandate, direct debit authority and/or standing instruction previously issued by me, pursuant to which the Company was authorised to effect automatic deductions.
3.3 This revocation is given in exercise of my rights and is to be treated as the withdrawal of consent and the termination of any continuing authority to process payments on my account.
3.4 I expect that the final permitted withdrawal in respect of my account shall occur on or before ________, after which no further debit, charge or withdrawal whatsoever shall be made.
