1. This letter is issued as a formal written notice and demand that you immediately cease and desist from the pattern of harassment, intimidation and unwarranted conduct which you have directed at me, and which commenced on or about ________ and has continued to date.
2. The acts of harassment complained of comprise, but are not limited to, the following:

3. The said conduct is unwarranted, unwelcome, unacceptable and has become unbearable to me. Your actions have caused, and continue to cause, me the following injury and damage:

4. TAKE NOTICE that your conduct constitutes an actionable wrong and, where applicable, a criminal offence. In particular, and without prejudice to other remedies available to me:
(a) your conduct is an infringement of my fundamental rights to dignity of the human person, personal liberty, private and family life, and freedom from inhuman or degrading treatment as guaranteed under Sections 34, 35 and 37 of the Constitution of the Federal Republic of Nigeria 1999 (as amended), enforceable under the Fundamental Rights (Enforcement Procedure) Rules 2009;

5. I HEREBY DEMAND that you, within ________ of receipt of this letter, and permanently thereafter:
(a) immediately and permanently cease and desist from the aforementioned conduct and from any further harassment, intimidation, surveillance, contact or communication with me, my family or my associates;
(b) refrain from publishing, repeating or causing to be published any defamatory or injurious statement concerning me;
(c) confirm to me in writing your agreement to comply with the foregoing demands.
