RE: FORMAL WARNING AGAINST WORKPLACE HARASSMENT

§1. Introduction. Further to the disciplinary meeting held with you on ________, this letter constitutes a formal written warning issued to you pursuant to the disciplinary procedures of ________ ("the Organisation"), your contract of employment dated ________, the Organisation's Anti-Harassment and Code of Conduct Policy, and the applicable provisions of the Labour Act, Cap. L1, Laws of the Federation of Nigeria 2004, and other relevant employment laws of the Federal Republic of Nigeria.

§2. Particulars of the Allegation. The Organisation has received and considered a complaint alleging that you engaged in inappropriate and unacceptable conduct amounting to workplace harassment towards ________ on or about ________. The specific particulars of the complaint against you are as follows:

§3. Prior Disciplinary Record. You were previously cautioned and/or warned regarding similar conduct, yet the unacceptable behaviour has continued. The particulars of your prior record in this regard are as follows:

§5. Formal Warning. Accordingly, you are hereby formally and unequivocally warned that:

§5.1. you must immediately cease all conduct of the nature described above;

§5.2. you are required at all times to conduct yourself in a professional, courteous and respectful manner towards all employees, clients and other persons associated with the Organisation; and

§6. Right of Response. Should you wish to make any representations or respond to this warning, you may do so in writing to the Human Resources Department within ________ of the date of this letter. Any such response will be duly considered in accordance with the Organisation's disciplinary procedures.

§7. Confidentiality. The subject matter of this letter, including the identity of the complainant, shall be treated as strictly confidential by all parties, and any act of retaliation or victimisation against the complainant or any witness will be regarded as a separate and serious act of misconduct.

§8. Acknowledgement. Kindly acknowledge receipt of this letter by signing and returning the attached copy to the Human Resources Department. A copy of this letter will be retained in your personal file.
