This letter is issued to you pursuant to the disciplinary procedure set out in your contract of employment dated ________ and the Employee Handbook/Conditions of Service of ________ ( o in conformity with the Labour Act, Cap. L1, Laws of the Federation of Nigeria 2004.

§ 1. Background

1.1 Following the meeting held with you on ________ (at which you were afforded the opportunity to be heard), the Company hereby formally notifies you that your performance and/or conduct in the discharge of your assigned duties has fallen below the standard reasonably expected of an employee in your position, particularly your failure to meet stipulated deadlines and the completion of assigned tasks.
