That I am a Director of ________ ("the Company"), a company duly incorporated under the Companies and Allied Matters Act 2020 with Registration/RC Number ________ and having its registered office situate at ________, and I am duly authorised by the Company to depose to this affidavit on its behalf.
  2. That by virtue of my position aforesaid, I am conversant with the facts herein deposed to.
  3. That the original Certificate of Registration/Incorporation of the Company bearing Registration/RC Number ________, issued by the Corporate Affairs Commission on the ________, has been lost.
