Re: Tenant's Notice to End a Periodic (Month-to-Month) Tenancy — Rental Unit located at ________

§ 1. Purpose of Notice. This letter constitutes my formal written notice to end the residential tenancy in respect of the rental unit described above (the "Rental Unit"), pursuant to and in accordance with the applicable Residential Tenancies Act (or the equivalent residential tenancy statute and regulations) of the Province of ________ (the "Act").

§ 2. Tenancy Particulars. The tenancy was created under a written tenancy agreement dated ________, under which the monthly rent is ________, payable on the ________ day of each month. The security deposit and/or pet damage deposit paid at the commencement of the tenancy totalled ________.

§ 3. Effective Date of Termination. The tenancy will end and the Rental Unit will be vacated on ________ (the "Termination Date"). I confirm that the Termination Date provides at least the minimum notice period required by the Act for the type of tenancy in question and, where applicable, falls on the day before the day the rent is ordinarily due.

§ 5. Move-Out Inspection. I request that we arrange a mutually convenient date and time to conduct the joint move-out (condition) inspection of the Rental Unit and to complete the prescribed condition inspection report in accordance with the Act.
