Dear ________,
Please accept this letter as my formal written Notice of Reservist Leave. I am being deployed on ________ in order to engage in a Canadian Forces operation inside of Canada.
As you are aware, this is a statutory leave of absence pursuant to the Canada Labour Code (R.S.C., 1985, c. L-2), under which I am entitled to up to 24 months of leave in any 60-month period. Based on my foregoing military engagement, I will require ________, with a possible variation. Should any changes arise, I will promptly notify you.
Evidence to take this leave of absence is enclosed with this letter to prove my entitlement under this leave, which includes the following documents: ________.
