RE: NOTICE OF RESERVIST LEAVE

Dear ________,

§ 1. Purpose of Notice. Please accept this letter as my formal written notice of intention to take a reservist leave of absence from my employment with ________. I currently hold the position of ________, and my employee or payroll identification number is ________. I am a member of the reserve force of the Canadian Forces and have been called out, or have volunteered, for service.

§ 2. Nature of Service. I am required to commence service on ________ in connection with the following operation or activity: ________. This service constitutes an eligible activity giving rise to a right to reservist leave.

§ 3. Statutory Entitlement. This is a statutory leave of absence. Where the federal jurisdiction applies, I am entitled to reservist leave pursuant to sections 247.5 to 247.7 of the Canada Labour Code (R.S.C., 1985, c. L-2) and the regulations thereunder, including the right to take leave for an annual deployment of up to twenty-four (24) months in any consecutive sixty (60) month period. Where the provincial jurisdiction applies, I rely on the reservist leave provisions of the employment standards legislation of ________, namely ________. This notice is given in accordance with the minimum notice period prescribed by the applicable statute, or as soon as is reasonably practicable in the circumstances.

§ 4. Period of Leave. Based on the foregoing service requirements, I anticipate that my leave will be of the following duration: ________________. This duration is subject to variation depending on operational requirements, and I will promptly notify you in writing of any material change to the anticipated start date, duration, or end date of my leave.

§ 5. Supporting Evidence. Enclosed with this letter is documentation evidencing my entitlement to this leave, which includes the following: ________.
