RE: REQUEST FOR REPAIRS — RENTAL PROPERTY AT ________

I am the tenant of the residential premises situated at the following address (the Premises) under a residential tenancy agreement commencing on ________:

I am writing to formally notify you, in your capacity as the lessor/landlord of the Premises, of a defect that requires repair, and to request that you arrange for the necessary repairs to be carried out in accordance with your obligations under the residential tenancy legislation applicable in ________.

This defect first became evident on or about ________. It was not apparent at the commencement of the tenancy or during my initial inspection of the Premises and has since become evident.
