§ 1. Purpose of This Notice. This letter constitutes formal written notice (this "Notice") that the Sender does not intend to renew, extend, or continue the contract identified below (the "Contract") beyond its current term, and that the Contract shall terminate upon the expiration of its present term in accordance with its terms.

§ 2. Identification of the Contract. This Notice relates to the following agreement:

§ 3. Current Expiration Date. The current term of the Contract is scheduled to expire on ________ (the "Expiration Date").

§ 4. Compliance with Notice Period. Under the terms of the Contract, written notice of non-renewal must be provided at least ________ prior to the Expiration Date. The Sender provides this Notice within, and in full compliance with, that required notice period.
