Re: Notice of Complaint and Demand to Remedy Conditions at ________ (the “Premises”)

Dear ________:

§ 1. Tenancy. I am your tenant in lawful possession of the Premises located at ________, pursuant to that certain written lease agreement dated ________ (the “Lease”). I am current in my rental and other obligations under the Lease, except as may be excused by the conditions described herein.

§ 2. Nature of the Complaint. I am writing to provide you with formal written notice of the following defect, condition, or breach affecting the Premises:

§ 3. Duration and Effect. This condition has materially interfered with my use and quiet enjoyment of the Premises for approximately the past ________ days. The condition affects the habitability and/or my safe and reasonable use of the Premises.

§ 4. Prior Notice. As you are aware, I previously brought this matter to your attention on ________ by means of ________. To date, the issue has not been resolved, despite the passage of a reasonable period of time.

§ 8. Demand and Cure Period. Please take whatever steps are required to resolve this matter promptly so that I may continue to enjoy the Premises as provided under the Lease and applicable law.
