RE: NOTICE OF TERMINATION OF LEASE AND DEMAND TO VACATE AND PAY
Dear ________,
§ 1. The Leased Premises. This notice pertains to the residential/commercial premises situated at:
________ (the “Leased Premises”).
§ 2. The Lease Contract. Your lease over the Leased Premises commenced on ________ and is set to expire on ________, at a monthly rental of ________ Pesos (₱________), payable in the manner stipulated in the Contract of Lease.
§ 3. Ground for Termination. Pursuant to Article 1673 of the Civil Code and the stipulations of the Contract of Lease, I hereby exercise my right to judicially or extrajudicially terminate the lease prior to its expiry by reason of your breach of the conditions thereof, particularly the following:
