Dear Sir/Madam,

§1. Introduction. I, ________, write further to the cancellation of ________ flight number ________ (the "Flight") to assert my statutory rights and to request reimbursement of the cost of the ticket together with the compensation to which I am entitled under Regulation (EC) No 261/2004 of the European Parliament and of the Council of 11 February 2004 (the "Regulation"), as given effect in Ireland by the European Communities (Compensation and Assistance to Air Passengers) (Denied Boarding, Cancellation or Long Delay of Flights) Regulations 2005 (S.I. No. 274 of 2005) (as amended).

§2. Particulars of the Flight. The Flight was scheduled to depart on ________ at ________ from ________, arriving at ________.

§3. Particulars of the Booking. I booked the ticket(s) on ________, under booking reference ________, in the name(s) of ________, comprising ________ passenger(s).

§4. Notice of Cancellation. I was notified of the cancellation of the Flight on ________, being only ________ (________) hours before the scheduled departure. Pursuant to Article 5 of the Regulation, where a passenger is informed of a cancellation less than the prescribed period before the scheduled time of departure and is not offered re-routing within the permitted timeframes, the operating air carrier is liable to pay compensation under Article 7 unless it can demonstrate that the cancellation was caused by extraordinary circumstances which could not have been avoided even if all reasonable measures had been taken.

§5. Consequences of the Cancellation. As a direct consequence of the cancellation of the Flight, I was obliged to take the following measures and incurred the following expenses:

In accordance with Article 8(1)(a) of the Regulation, I hereby exercise my right to reimbursement in full of the cost of the ticket within seven (7) days, at the price at which it was purchased.
